This is a Japanese example, but I can think of an anime series whose initial English sub accidentally created an incestous relationship as part of a character’s backstory; Zelgadis from Slayers, uncertain of his exact relationship to his master Rezo, speculates that the latter was his grandfather or great-grandfather—and somewhere that “or” became an “and!”
